A Growing HIV Challenge in Mizoram
In a concerning development, the Mizoram State AIDS Control Society (MSACS) has highlighted an increase in the number of men having sex with men (MSM) in Mizoram, posing a significant challenge in the fight against HIV. This issue holds particular importance for North East India, a region that has been grappling with HIV prevalence.
Unregistered MSM Pose a Significant Risk
According to MSACS, 1,105 MSM have been registered and are receiving counselling services. However, officials estimate that this figure represents only a fraction of the actual number. Many MSM avoid registration due to fear of identity disclosure, which could lead to dangerous oversights in HIV prevention efforts.
MSM in Urban and Rural Areas
Contrary to common perception, MSM are not confined to urban areas in Mizoram. They are present in rural villages as well, indicating a higher actual number than those currently registered. This dispersed population makes it challenging to reach and provide necessary services.
HIV Prevalence and Transmission in Mizoram
As of 2026, a total of 33,781 HIV cases have been reported in Mizoram since 1990. Between April and November 2025 alone, 3,257 new infections were detected, reflecting a 2.24% positivity rate. Sexual transmission remains the primary mode of HIV transmission, accounting for 70.04% of cases.
HIV Prevention Efforts in Mizoram
Despite the challenges, Mizoram ranks third in the country in terms of HIV prevention efforts. The State has 14 antiretroviral therapy (ART) centres, with 18,231 people currently on ART. However, 1,169 individuals who had registered for treatment have discontinued it, while more than 7,000 HIV-positive persons are yet to begin ART.
Implications for North East India and Beyond
The rising number of HIV cases among MSM in Mizoram underscores the need for comprehensive and targeted HIV prevention strategies in the North East region. Addressing the fear of identity disclosure and ensuring access to counselling and treatment services for all MSM, regardless of their location, is crucial.
